Tirzepatide/Cyanocobalamin Weight Loss Plan Available in Early, Texas (COMPOUNDED MOUNJARO)

Tirzepatide is a novel dual glucose-dependent insulinotropic polypepetide (GIP) and glucagon-like peptide-1 (GLP-1) receptor agonist.  It is composed of 39 amino acids and is an analog of the gastric inhibitory polypeptide.  It enhances the insulin response, suppresses glucagon secretion, promotes satiety, and improves insulin sensitivity.  Tirzepatide works by helping the patient feel full on less food and slowing the passage of food through the digestive tract.

Cyanocobalamin (Vitamin B12) is a water-soluble vitamin that plays essential roles in red blood cell creation, cell metabolism, nerve function and production of DNA.  It also works as a coenzyme in the conversion of protein and fat into energy.

Tirzepatide and Semaglutide are both medications used for the treatment of type 2 diabetes and obesity, but they have some differences in their mechanisms of action and clinical efficacy.

1. Mechanism of action: Tirzepatide is a dual glucose-dependent insulinotropic polypeptide (GIP) and glucagon-like peptide-1 receptor (GLP-1R) agonist. It activates both GIP and GLP-1 receptors, leading to increased insulin secretion, decreased glucagon secretion, delayed gastric emptying, and reduced appetite. On the other hand, Semaglutide is a GLP-1R agonist that primarily acts on GLP-1 receptors to stimulate insulin secretion, inhibit glucagon secretion, slow gastric emptying, and promote satiety.

2. Efficacy in glycemic control: Both Tirzepatide and Semaglutide have shown significant improvements in glycemic control. However, Tirzepatide has demonstrated superior glycemic control compared to Semaglutide in clinical trials. In a head-to-head study, Tirzepatide achieved greater reductions in HbA1c (a measure of average blood glucose levels) compared to Semaglutide.

3. Weight loss effects: Both medications have also demonstrated great weight loss benefits. However, Tirzepatide has shown more pronounced weight loss effects compared to Semaglutide. In clinical trials, Tirzepatide led to greater reductions in body weight and waist circumference compared to Semaglutide.

Common side effects of tirzepatide may include:

1. Gastrointestinal symptoms: Nausea, vomiting, diarrhea, abdominal pain, and indigestion are commonly reported side effects. These symptoms usually occur at the beginning of treatment and may improve over time.

2. Hypoglycemia: Tirzepatide can lower blood sugar levels, which may lead to hypoglycemia (low blood sugar). Symptoms of hypoglycemia may include sweating, shakiness, dizziness, hunger, fast heartbeat, and confusion.

3. Injection site reactions: Redness, itching, swelling, or pain at the injection site may occur.

4. Loss of appetite: Tirzepatide may reduce appetite, leading to decreased food intake.

5. Fatigue: Some individuals may experience fatigue or tiredness while taking tirzepatide.

6. Diarrhea: Diarrhea can be a common side effect of tirzepatide.

7. Nausea and vomiting: These gastrointestinal symptoms may occur as a side effect of tirzepatide.

8. Headache: Headache can occur in some individuals taking tirzepatide.

9. Upper respiratory tract infection: Common cold-like symptoms, such as a runny nose, sore throat, or cough, may occur.

10. Urinary tract infection: Some individuals may experience urinary tract infections while taking tirzepatide.

*It is important to note that this is not an exhaustive list of side effects, and other rare or severe side effects may occur.

Tirzepatide may not be suitable for individuals with certain medical conditions or taking specific medications due to potential contraindications. Some possible contraindications for tirzepatide include:

1. Hypersensitivity: If you have a known hypersensitivity or allergy to Tirzepatide or any of its components, it is contraindicated to use this medication.

2. Medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2): Tirzepatide is contraindicated in individuals with a personal or family history of MTC or MEN 2, as it may increase the risk of developing these conditions.

3. Diabetic ketoacidosis (DKA): Tirzepatide should not be used in individuals with a history of DKA or current DKA, a potentially life-threatening condition characterized by high blood sugar and ketone buildup.

4. Severe gastrointestinal disease: If you have a history of severe gastrointestinal disease, such as gastroparesis or inflammatory bowel disease, Tirzepatide may not be appropriate due to its potential gastrointestinal side effects.

5. Pancreatitis: Tirzepatide is contraindicated in individuals with a history of pancreatitis or a current episode of pancreatitis.

6. Pregnancy and breastfeeding: The safety of Tirzepatide during pregnancy and breastfeeding has not been established. Therefore, it is generally not recommended for use in these situations.

Other relative contraindications also exits, such as gallbladder disease or diabetic retinopathy.

It's important to discuss your medical history, current medications, and any potential contraindications with your healthcare provider before starting Tirzepatide or any other medication.

Once you are evaluated by one of our providers, the medication will be ordered through our practice and shipped directly to our office for pick-up.

For your initial visit, you will need to schedule an appointment with our office. Upon arriving for your appointment, you will have your consultation with our Nurse Practitioner who will use her discretion (based on the FDA recommendations for treating obesity) to determine if Tirzepatide is a safe and good option for you. She will carefully screen you by evaluating your past medical history, any current medications you take, your vital signs, and your body mass index (BMI).

If you are a good candidate for this plan, you will be scheduled for follow-up weigh-ins, so we can continue to properly monitor your treatment and refill your Semaglutide prescription. 

You are NOT a good candidate for Tirzepatide if you have any of the following:

Hypersensitivity,  Medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2), Diabetic ketoacidosis (DKA), Severe gastrointestinal disease, Pancreatitis and/or Pregnancy, planning to become pregnant or breastfeeding.

Tirzepatide is injected under the skin (subQ) in your stomach, thigh, or upper arm one time each week, on the same day each week at any time of day.

How much Tirzepatide do I inject?

Patients will begin with the lowest recommended dose titrating up as tolerance grows. 

Injections will be administered once weekly.
